When a patient from Corporate Dental Practices ( CDP ) comes to your clinic you are required to do the following: 1. Take the following details from the patient : a. Name of patient. b. Company where he is working. c. Branch / location where he works. d. Registration number. 2. Call us on 9867714441 or 022 28647182 with the details. We shall check our database and confirm the registration of the employee.
3. Once we confirm the registration, you have to check the ID proof of the person. After confirming the ID proof you can commence the treatment. 4. Once the treatment is completed, you have to give the patient a bill for the amount collected from the patient. 5. Now you shall have to make the entry into the data entry sheets that we are sending you. Please make sure that your writing is legible and write in black ink. 6. At the end of every month, you have to send us the data entry sheets either by fax or by courier. 7. If you have any questions, please get in touch with us and we shall be happy to assist you. PLEASE NOTE - The treatments covered under the Corporate Dental Practices ( CDP ) package shall be dispensed according to the charges prescribed in the rate card.
- The panel dentist shall collect the payment for treatments directly from the patient and shall give a bill for the same.
- Corporate Dental Practices ( CDP ) or the corporate shall not be liable for any non-payment of dues.
- The panel dentist shall be solely responsible for any negligence, deficiency in services or imperfections in rendering of the treatments to the patients.
- The panel dentist will indemnify Corporate Dental Practices ( CDP ) for any loss or damages occured to Corporate Dental Practices ( CDP ) on account of any dereliction of duty or negligence on your part.
- The treatment package under Corporate Dental Practices ( CDP ) is applicable to the employee of the partnering company and their immediate family comprising of the spouse, children and one set of parents of the employee.
- The panel dentist shall maintain a log of all patients treated under the Corporate Dental Practices ( CDP ) scheme and shall provide the same to Corporate Dental Practices ( CDP ) on a monthly basis.
- The disciplinary committee reserves the right to include / exclude any dentist from the Corporate Dental Practices ( CDP ) panel.
- The panel dentist will be required to give a one month advance notice to Corporate Dental Practices ( CDP ) before resigning from the panel.
