Corporate Dental Practices ( CDP ) shall provide the company with a list of all panel dentists.

Only the treatments mentioned in the rate card are covered by Corporate Dental Practices ( CDP ). All other treatments shall be provided by the panel dentist as per the rate mutually agreed upon by the respective panel dentist and the employee.

The company shall circulate the offer along with the rate card of Corporate Dental Practices ( CDP ) to all their employees, who can avail of the services of any panel dentist of Corporate Dental Practices ( CDP ) at their own discretion and free will. Company does not take any financial or other responsibility on behalf of its employees.

The company understands that while maximum care has been taken while appointing experienced and qualified dental surgeons on its panel, Corporate Dental Practices ( CDP ) does not take any responsibility for any damage caused to the patient on account of any shortcomings, negligence or deficiency in services in the working of any panel doctors.

Company shall allow Corporate Dental Practices ( CDP ) to organise dental awareness seminars and check up camps at the factory or offices of the company, free of charge, at approved place and time.

Corporate Dental Practices ( CDP ) reserves the right to revise its rate card on a periodical basis.

Company shall keep Corporate Dental Practices ( CDP ) updated with the revised employee list from time to time.

Company may authorise Corporate Dental Practices ( CDP ) to use the name of the company and the tie up with Corporate Dental Practices ( CDP ), after approving the promotional matter which will be signed off. However the company does authorise Corporate Dental Practices ( CDP ) to mention the tie up on its website.

Corporate Dental Practices ( CDP ) as well as the company are at liberty to terminate this arrangement after giving 30 days prior notice.

No contractual obligations or liabilities whatsoever shall be cast on the company or its employees.
